Completion and submission of this form confirms that my
organization supports the San Diego County Childhood Obesity
Initiative, a public/private partnership whose mission is to reduce
and prevent childhood obesity in San Diego County by creating
healthy environments for all children and families through
advocacy, education, policy development, and environmental
change.
The San Diego County Childhood Obesity Initiative is guided by
strategies identified in the San Diego County Childhood Obesity
Action Plan in seven domain areas. Please mark your domain
area(s) of interest and/or expertise.
